As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility and independence from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you'll empower individuals to overcome limitations and enjoy a better quality of life.
Responsibilities
Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain the highest level of physical function.
Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
Develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and the goals of the patient team.
Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
Document patient observations, interventions, and evaluations promptly and thoroughly.
Report patient progress to the physician, Clinical Manager, care team, patient, and family; submit all required evaluations, plans, progress notes, and discharge summaries.
Required Qualifications
Degree from an accredited Physical Therapy Program (approved by the APTA)
Current and unrestricted Physical Therapy license
Current CPR certification
Experience collaborating with a team of healthcare professionals
Preferred Qualifications
One year of experience as a Physical Therapist
Pay Range
$59.00 - $83.00 per visit/unit; $93,000 - $128,000 per year base pay.
Additional Information
TB Statement: This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB.
Driving Statement: This role is part of Humana's driver safety program and requires a valid state driver's license and personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage or whichever is higher.
